Root Cause Defined

A root cause is any cause that is causally connected to the defined problem that if acted upon will result in preventing the problem from occurring.

Root Cause Dictum

It is not the root cause we seek, it is effective solutions. Most other RCA methods waste their time trying to find root causes.

Root Cause Analysis (RCA) Defined

A Root Cause Analysis is any structured process used to understand the causes of past events for the purpose of preventing recurrence. To be classified as a Root Cause Analysis, the process must answer the following questions in the order stated below:

  1. What is the problem to be analyzed? (Problem Definition)
  2. What is the significance to the stakeholders? (Significance)
  3. What are the causal relationships that combined to cause the defined problem? (Problem Analysis)
  4. How are the causes interrelated? (Causal Chart)
  5. What evidence is provided to support each cause? (Cause Validation)
  6. What are the proposed solutions to prevent recurrence? (Corrective Actions)
  7. What assurance is provided that the solutions will prevent recurrence? (Solution Validation)

The Value of Evidence

Unique to the Realitychart is the inclusion of evidenced–based causes. When you can’t find evidence, or can’t find the next cause, embrace your ignorance and use a question mark. Evidence should be data we know from using our senses, not supposition. It supports the existence of the various causes. Without sensed evidence, we are subject to our own prejudices and preconceived ideas. It helps to be humble when pursuing causes and evidence. When you don’t know, admit it and use a question mark to signify this on the chart. If there is value in pursuing the unknown, then do so. If no value is perceived, you can make a conscious decision to stop.
