A root cause is any cause that is causally connected to the defined problem that if acted upon will result in preventing the problem from occurring.

A Root Cause Analysis is any structured process used to understand the causes of past events for the purpose of preventing recurrence. To be classified as a Root Cause Analysis, the process must answer the following questions in the order stated below:
The Value of Evidence
Unique to the Realitychart is the inclusion of evidenced–based causes. When you can’t find evidence, or can’t find the next cause, embrace your ignorance and use a question mark. Evidence should be data we know from using our senses, not supposition. It supports the existence of the various causes. Without sensed evidence, we are subject to our own prejudices and preconceived ideas. It helps to be humble when pursuing causes and evidence. When you don’t know, admit it and use a question mark to signify this on the chart. If there is value in pursuing the unknown, then do so. If no value is perceived, you can make a conscious decision to stop.
